The two things we most want to see - the Alhambra and the Sagrada Familia - are apparently the two hardest tickets to get, selling out weeks ahead with fiddly timed entries. Half the appeal of a tour is not having to wrestle with that. Does a guided trip actually take it off your hands?

Yes, a lot of Spain tours do handle the tickets for you, especially when the Alhambra or Sagrada Família is listed as an included sightseeing visit. But every tour is different, so I would ALWAYS check the What’s Included / What’s Not Included section before booking rather than assuming admission is covered.
I actually planned my own Spain trip, and I’m visiting the Alhambra in two weeks. The regular admission tickets that include the Nasrid Palaces sold out at least two months ago! I’ve noticed similar availability issues with the Sagrada Família, so I’m VERY glad we already had both booked 
If you’re looking at a tour where those admissions are included, I’d still book the tour at least a few months ahead if you can, especially during peak tourist season. This is honestly one of those situations where I completely understand the appeal of an organized tour — having someone else secure those timed entries takes away a pretty stressful part of the planning!
I find with any “hot ticket” like these two sights, it’s always a good idea to explore the idea of joining a tour. That way, you can check with the operator that they include this, have that peace of mind and don’t have to worry about another thing you have to check on your list before you depart.
